Welcome to our GUIDE program—Guiding an Improved Dementia Experience. We are grateful to be among a few provider groups nationally selected by Medicare to offer this new comprehensive dementia care program. At Healthspan Partners our mission is to improve quality of life for people living with dementia while supporting their caregivers.

The GUIDE program is a medically managed care management program that provides comprehensive assessment and care planning, care coordination, caregiver support, and respite (personal care) services at no cost to Medicare beneficiaries. We serve people with diagnosed or suspected dementia and their caregivers. In the GUIDE program, a caregiver is any unpaid family member or friend who provides any level of care or assistance to someone with dementia.

No Cost Personal Care / Respite
Eligible individuals may receive personal care or respite services at no cost. This gives caregivers a break while ensuring loved ones receive quality care.
