
Welcome To Our GUIDE Program
Guiding an Improved Dementia Experience. We are grateful to be among a few provider groups nationally selected by Medicare to offer this new comprehensive dementia care program. At Healthspan Partners of our mission is to improve quality of life for people living with dementia while supporting their caregivers.
The GUIDE program is a medically managed care management program that provides comprehensive assessment and care planning, care coordination, caregiver support, and respite (personal care) services at no cost to Medicare beneficiaries. We serve people with diagnosed or suspected dementia and their caregivers. In this program, a caregiver is any unpaid individual, such as a family member or friend, who provides care or assistance to someone with dementia.

Meet Our Team
Adam Perry, MD
Founder of Healthspan Partners
Adam Perry, MD is a fellowship-trained geriatrician and founder of Healthspan Partners of SWFL, a novel dementia care practice administering the Medicare GUIDE program. His career is dedicated to the design and dissemination of new models of care that enhance quality of life for older adults with complex care needs. He is a member of both the Milken Alliance to Improve Dementia Care and the West Health Center of Excellence for Telehealth and Aging. Previously he was a Fellow with the John A. Hartford Foundation Health and Aging Policy Program and Core Faculty with the Geriatric Emergency Department Collaborative. He has published multiple manuscripts across medical specialties and presents regularly to state and national conferences on emerging models of care for older adults with dementia and/or complex care needs. He is a graduate of the University of Pittsburgh School of Medicine with post-graduate training at University of Pennsylvania Lancaster General Health and Thomas Jefferson University Hospital.

Deann Marasco
Director of Operations and Development and HUB Administrator at Healthspan Partners of SWFL
Deann Marasco, Director of Operations and Development and HUB Administrator at Healthspan Partners of SWFL, brings extensive experience in healthcare advocacy and strategic program development. With a background in Gerontology from the University of South Florida, she has held leadership roles in large health systems, Westcoast Brace & Limb, USF Health, and most recently, more than sevens years at the Alzheimer’s Association.

Her volunteer work with the Florida National Guard’s Family Readiness Group earned her the Florida Commendation Medal for sustained meritorious performance.
At Healthspan Partners of SWFL, Deann will lead efforts with partnership organizations to improve outcomes for those living with dementia and supporting the care ecosystem.
Patricia Angelucci, GNP-BC, APNP
Patricia Angelucci is an Adult–Geriatric Nurse Practitioner dedicated to improving the quality of life for older adults and supporting the families who care for them. She is driven by a deep passion to serve seniors with empathy, compassion, and respect, recognizing the unique medical, emotional, and social needs that often accompany aging.Board-certified in geriatrics, Patricia brings specialized expertise in geriatric medicine and the complex health challenges faced by older adults. Her clinical focus includes palliative medicine, hospice care, chronic care management, and dementia care. Through this work, she helps patients and their loved ones navigate difficult medical decisions while maintaining dignity, comfort, and quality of life.Patricia is committed to a patient-centered approach that values clear communication, thoughtful care planning, and collaboration with families and healthcare teams. Her goal is to ensure that every patient receives compassionate, individualized care that honors both their medical needs and personal values.

Adult-Gerontology Primary Care Nurse Practitioner
Nicole D-Amico-Shaal, MSN, APRN, AGPCNP-C, is an Adult-Gerontology Primary Care Nurse Practitioner dedicated to delivering compassionate, patient-centered care in both inpatient and outpatient settings. She specializes in supporting older adults and their families as they navigate complex medical conditions, helping them understand treatment options, manage symptoms, and maintain the highest possible quality of life.
Nicole is known for her thoughtful, collaborative approach and her commitment to building strong relationships with patients, caregivers, and interdisciplinary healthcare teams. She believes the best care begins with listening closely to patients’ goals and ensuring care plans reflect their individual values and needs.
With a strong clinical foundation in nursing and advanced practice, Nicole is passionate about patient advocacy and providing high-quality, evidence-based care that emphasizes dignity, comfort, and respect throughout every stage of health.
Laurel Powers, LCSW
Licensed Clinical Social Worker
I am a Licensed Clinical Social Worker (LCSW) with more than 15 years of experience in hospital-based care management and care coordination for adults with complex medical and social needs. I earned my Bachelor’s degree in Psychology from the University of South Florida and my Master of Social Work from the University of Central Florida. Since 2008, I have specialized in hospital social work across orthopedics, neurology, trauma, and complex discharge planning, with a strong focus on supporting aging adults and their families through challenging healthcare transitions. I became licensed in 2014 and also provide clinical supervision to social workers pursuing licensure. My work centers on helping families navigate care decisions, preparing caregivers for the responsibilities ahead, and ensuring safe, coordinated transitions across care settings. I am deeply committed to communication, collaboration, and continuity of care to improve outcomes for older adults.
