Eligibility for GUIDE Model Participation

Beneficiars may have mild, moderate, or severe dementia.

Beneficiaries must be enrolled in "Traditional Medicare", with Medicare Parts A and B.

Beneficiaries enrolled in Medicare Advantage, a Special Needs Plan, or PACE are not eligible.

People enrolled in the Medicare hospice benefit are not eligible.

Participants may live in a home, apartment, or senior living environment such as Assisted Living or Memory Care. Residents of Long-Term Nursing Homes are not eligible.

Participants must live in the service area of their GUIDE program provider.

Beneficiaries may only receive services from one GUIDE Model Program.
